No Show/Cancellation Policy: As the wait time to be seen in consultation can be long, each appointment is very important. If you cannot make your appointment time, please call ahead to let us know, and if you cannot make the appointment at all, please advise us at least 24 hours in advance so that we may fill the time. To cancel or re-book, call our clinic or use the cancellation form.

Referral Process: Please use the Facilitated Access to Specialized Treatment (FAST)
Adult Urology Referral form
as the cover page for your referral, and fax the complete referral to 1-833-627-7023.

Note: referrals are only accepted from a referring office (including general practitioner, family physician, or other specialty).

The referring office will receive a receipt of referral acknowledgement from the Alberta Facilitated Access to Specialized Treatment (FAST) program within 48 business hours (hours of operation: 8:00 am – 4:00 pm Monday through Friday; closed on statutory holidays). Once received, the referral will be forwarded to an appropriate provider. Triage will be completed by the assigned provider within one to two weeks, as per CPSA standards.
